Pet of the Month
Does your pet deserve to be Compassionate Care Animal Clinic's pet of the month? We want to know why your pet is special to you! Our Pet of the Month entry form can be completed online below. If you prefer to mail or submit in person, please click here to print the entry form.
Entry is limited to current clients and pets of Compassionate Care Animal Clinic. You may enter any species of pet – dog, cat, rabbit, guinea pig, - we love them all! Only enter once. Each month your entry will still be valid and considered for upcoming features. By entering you give us permission to post your photo on our website, Facebook, in our office and in future marketing materials. You will be notified if your pet is selected. There are no prizes, just bragging rights! The first featured pet will be announced May 6.
By postal mail or in person: Submit your pet's photo and the form below. Send to Compassionate Care Animal Clinic, 150 Victory Lane, Elkhart Lake, WI 53020, drop off in person: 150 Victory Lane, Elkhart Lake, WI. Pictures will not be returned to the owner, so please make a copy for submission.
By email or Facebook Messenger: Submit your pet's photo and the form below. Email to office@ccacpetvet.com. Pictures need to be in .jpg format. Limit two photos and up to 5 MB total. Find us on Facebook by searching Compassionate Care Animal Clinic.
